Paisley 15th. February 1778


in the Month of August last -------
Mr. Orr observed an Ulcer upon the in¬
side of the
Prepuce near it's junction
with the penis -- an Affair he had had
with a girl a week before was con¬
sidered as the Cause, for there was no
reason to suspect any preceding infec¬
tion
-- Mr Orr trusting to it's appa¬
rent insignificancy neglected the Com¬
plaint
ailment and it was ↑two or↑ three weeks
longer before he was persu↑a↑ded to take
Medicine and mercury was ↑then↑ given in
such quantity as to affect his breath
at the same time that it admitted
of his going Abroad and following
his usual employments -- this Course
was continued for two months when
the Ulcer healed - but our attention
was directed to a more serious pp
symptom for he now Complained of
his throat which he said had been
ailing for some time tho' he had
taken little or no notice of it and upon
inspection a large whitish ulcer was
discovered upon each
Tonsil -- he
was now Confined to his Chamber and
Mercury administered more freely so

as to raise a pretty Copious salivation
which after being kept up for a month (↑three weeks↑)
longer was suddenly stopped in Conse¬
quence of a severe purging, accom¬
pained with gripes, Tenesmus and
Strangury, brought on by exposing
himself incautiously to Cold -- the
ulcers having disappeared some days
before this attack, we were in hopes
that all was over, but found our¬
selves disappointed; for in a few
days the Whitishness upon the Ton¬
sils
became again apparent and
ended in ulcers as bad or worse than
the former being now accompained
with another upon the Velum close
to the Uvula -- Mercury was a¬
gain had recourse to, and the ul¬
cers
again disappeared and the
salivation Continued till all danger
of a relapse was supposed to be entire¬
ly removed -- but here too we were
again deceived for three new Shankers
broke out upon the inside of the Pre¬
puce
, his Throat becoming at the same
time inflamed, dry and uneasy --


Once more then we betook ourselves

to the Mercury and a gentle saliva¬
tion being raised
was persisted in till
these ulcers together with the inflam¬
mation
, driness and soreness of the
throat and every suspicious symp¬
tom were gone and all possibility
of a return was believed to be over,
nor was it stopped till about ten
days ago when a return of the
purging, laid us under a necessity
to desist -- now at last we were
sure of having got to an end of
our labour! -- but we were wrong
for contrary to all expectation
within these six days the inflam¬
mation of the
Fauces and Velum
has returned and upon friday last
another shanker has appeared upon
again upon the inside of the prepuce
a new Complaint has likewise arisen
of a pain in the fore and upper part
of the Thyroid
Cartilage which is
uneasy to the touch and dm in the
action of deglutition but his Voice
is not affected --


His pulse has been all along
Calm and regular
- his appetite is

likewise good and as he has been in¬
dulged for some time past in a Diet
more nourishing than was allowed
at the beginning I cannot say he
is very much exhausted or excessive¬
ly weak
--- we have begun our
old Course but his spirits are sadly
dejected
by the returns of this Vexa¬
tious disease; and tho' it appears
necessary to me necessary to continue
a gentle salivation for a considerable
time even after every symptom has
disappeared I dare scarce propose
it to him harrassed and tantalised
as he has been with the false prospects
of a Cure and repeated a Courses of
medicine he has already run thro' -
it is with pleasure therefore I draw up
it at Mr. Orr's desire his Case for Dr.
Cullen's inspection - and if mer a Course
of Mercury appear necessary it is begged
the Dr. will particularly specify the
time of it's Continuance ---


Rubbing has been the mode in which
Mercury has been administered to Mr.
orr, every other method being found inef¬
fectual - for owing to an uncommon irrita¬
bility of his
gutts, when given inwardly
it never fails to bring on ↑a↑ purging about to be
stopped by opiates and even Decoctions of sarsa¬
parilla
or the woods disorder him.
