Whitehaven August 1st., 1775
Sir,


I have desired a Patient of mine to consult
you he is aged 44 years, has lived Temperate, of a
sanguine habit of Body, before he was twenty he
had in his neck an ulcer supposed to be Scrophulous
which healed up then and in his twenty fourth year
had a fit of the Rheumatism which confined him to
his Bed two or three Weeks. For many years subject
to a griping in his Bowels, and of late in his Stomach,
frequent Nausea but no swelling in his Belly or
any fixed pain ↑but↑ an aching as he terms it, the Wind
which he rifts up is hot, he belches his meat up
some days in Mouth fulls and once or twice a
Week has a large puking. He has a hard ridge of
the size of a Finger that runs a cross the Hypochondriac
region
just below the skin sometimes acid eructation,
little appetite four Months ago he lost his Flesh very
much, and is much depressed in Mind believing his
Disease incurable; he had before I attended him too
fits of the Gout in his great Toe the last Fit was too
years ago & since my attending had a Fit in Aprile #[Start of margin text]# a second in June they lasted about six or seven Day[s] {illeg} [h]e made no complaint of hisAbdomen during the Fits,[End of margin text] I
must not omit to acquaint ↑you↑ that the above swelling
does not appear to have any connection with any
of the Viscera this is rubed twice a Day with the
Ol. Camph. He was sweat'd a few times with Pulv.
Dover.
which palliated his Complaints. He has
taken Magnes. alb. to no purpose. --- The Cort.
Peruv.
et Trifol. aquat. hurt his Stomach, his ap¬
petite decreased
& his Spirits more depressed. Or did
he find any use from the [Bol. e Pip. Pedulium?] --
Sem. Sinap.-- Ol Terebinth.-- Spt.Æther.--
Mixt. Salin. -- Spt. fœtid. vol. was very disagreeable.

Saltzrs Water did not sit well on his Stomach.
He thought himself worse in his Belly after
a gentle puke of Ipecac. about ten Days ago.
Dr Cleghorn who at our Patients desire I con¬
sulted in Aprile last ordered the Cortex infused
in Aq. Calc. with Drops of Tinct. Guaiac. vol.
Flor. mart. & a little Tinct. Thebaic. occasionally
& to keep his Bowels soluble with the Aloetic
Pill
which produced no apparent effect. At times
he will puke Madera Wine or Brandy at other
times they will sit easy. He is regular in
going to stool unless he takes Anodines. He
takes milk morning and evening, and at noon
to Diner Flesh meats. Rides out on Horse back
every Day when the weather permits. Supposing
some Gouty humour might be lodged in the
Viscera I ordered drops of Tinct. Guaiac. vol.
Vin. Antimon. and a small quantity of
Tinct Thebaic. in a Julep composed of Conf.
Cardiac.
Julep. e camphor. Sal. C.C. Tinct
Flor. mart. & Lixis. Sapon. & Aloetic Pills
to be taken occasionally which in general has
agreed the best with him.

P. S. I am sorry I could not make our Patient
advance a larger Fee. 1 -- Our old Patient that
you was so kind as to Prescribe for in the Megrim
is just the same. 2


N. B. Our Patients Pulse is natural, and he has
for a long time wore a Flannel Shirt, & refraind
from every thing acid in his diet & yet at times
what he throws up is very sour.

Since I wrote the above, I have felt his Abdomen again; &
find the swelling to be now situated just above the navel,
inclining to the right side, of the size of the palm of the hand,
but not by much so thick; it feels quite hard as if the
muscular part of the Abdomen in that particular
part was in a spasmodic state no pain in it nor
is it discoloured
. He says when his Belly is at the
worst, hard Lumps will rise in various parts,
which disappear again, as he grows easier in a few
minutes. He can scarce take flesh meats at present.


They have no Hereditary Diseases in their Family.


I have enclosed in this Letter a Bill for one Guinea.
