Berwick 15 March 1774
Dear Sir


I am going to relate you a Case which I think is
perplexed and complicated- Jane Dickenson a servant of
Sir Thomas Haggerstons about thirty years of age, has had ↑for upwards of two years↑ at
the time of menstruation (which generally lasts 4 or 5 days)
very violent pains about the bottom of her belly & genital
parts
attended with sickness and vomiting, the quantity
discharged very small compared with what she used
formerly to have. She consulted you when you were last
at Haggerston when you ordered her Tinct. thebaic every
night during the pains, this medicine she thought releived
her much. She has likewise for about three years had
gravelly complaints such as pains in her right kidney,
Vomitings, voiding sand in her urine &c.


About three months ago she complained of pain in her
stomach, sickness, want of appetite, thought her stomach
swelled and at the same time complained of pains in her
bowels, The night she thought these complaints in her
stomach subsided she complained of a violent pain in
making water
especially in the last drops and was seized
with a purging in which she voided by stool stuff
resembling matter, Fat, or Corruption, and along with
these stools black sand, at the same time she had the strangury
and a discharge of matter from the Vagina which stained

her shift yellow like the discharge from a Gonorrhea
all these complaints continue much the same together
with the pain in her right kidney, she had her menses
last week and what was discharged she says was like
matter and very fœtid, These complaints have all along
been attended with a thirst, her pulse has always been
and is yet what we may call very good, since these last
complaints she has had a better ↑appetite↑ for her victualls than formerly
when her complaints were in her stomach-


I have enclosed you a Guinea Note and will be
glad if you will please to advise me what method you
would have me take with this patient.
