been for these many years distress & periodically, with
lowness of Spirits &c. which he will particularly describe
A year Ago or little more now was sized with pains in his
back difficulty of breathing Dysury for which he was
bled, cooling diuretic medicines were used which he was
releived from, at last appearances of the Gout appeared
in his foot & while that continued his mental complaints
kept off but gravelish urine with red sand difficulty in
voiding
&c. flying pains thro' his body particularly in his
side but relieved by blisters, his recent alment is --
of the Rheumatic tribe in the Haunch, thigh, & leg. --
He is of a Gross habit which you will see, is a sober
& temperate man -- And inter Nos encourage him
I have only given you the outlines of his complaints but
he will himself inform you more fully. And I am

Sir
your most Obedient humble Servant
