A. B. Aged 22 Brown complexion hair & eyes
About 2 Months ago was seized with pains
in the Præcordia, & Nausea, which were removed by the
Exhibition of 2 chamomile vomits -
In about 10 days thereafter the Nausea returned
with headach, & soon after, his sight was so much affec¬
ted as made him judge very improperly of the objects
around him - At this time he complained of ge¬
neral bodily debility & his Pulse was about 100
small & feeble & the pain fixed in both temples
Afterwards & now the pain seizes him in the forehead
- Objects formerly appeared to him much increased
in size - At one period he had double vision, which
went off after vomiting - The headach is always
most severe in the morning about 2 or 3 O clock, & con¬
tinues till towards noon, but commonly goes off
or is greatly better about 5 O clock -
His complaint in general followed this course regu¬
larly, untill he had taken some Peruvian bark, when the

headach & Nausea disappeared for one, two, three, or four
days; afterwards returning (it is presumed oweing to some
irregularity or neglect) & continuing for one, two, or three days.
The Nausea is now gone off - The headach much
abated & less intense & generally goes off about noon;
Appetite better, sleeps more & sensible of refreshment,
Double vision gone, as also the deceptio visus -
Objects now appear to him of the natural size, & in
general he thinks himself considerably stronger and
getting well -
Had an irregular ague at Chatham in 1783 and
has been more or less affected with it every Spring
& Autumn since - Has (as most people at his time
of life) been exposed to irregularities
Belly regular, Tongue clean. Pulse not 75 & equal
Having in view the frequent returns of his Aguish
complaints & the extreme bodily debility (the last now
in a good measure relieved) induced by habits - Gentle

vomits & cathartics to relieve the Stomach and Prima Viae, there¬
after Peruvian bark in different forms, so as to be acco¬
modated to the patients liking were given, with
Elixir Vitrioli & a glass of red wine two or three times
a day along with the cortex - A blister was lately
applyed to his neck & his head shaved - wears flannel
next his Skin sleeps upon a Mattrass or hard bed & is
directed not to indulge himself with lying too long
a bed - to take moderate exercise on horse back or
on a spring board; which he complies with
Cork soaled shoes have been ordered & to avoid damp &
wet - Nourishing Diet with a few glasses of Claret
after Dinner has also been advised
Such Directions as Dr. Cullen sees necessary to give
