Doctor

A patient of mine, a gentleman aged
38, fair complexioned, & a full habit of body,
was seized last spring in April after expos¬
sure to the north & east winds which then pre¬
vailed with great pain in both Jaws, on the
left particularly on the upper, extending
to the Crown very often.

The complaint was particularly severe
in the night time, & found no relief from
topical bleedings, blistering & the Bark. ---

The farthest upper grinder was down, which
gave relief, but the disease has never been
entirely removed, twitches of pain being
occasionally felt, upon the application of
hot or cold liquors, or upon a cold wind.

This disease took place when he had
been taking mercury, & his mouth was
slightly affected by it, at the time of the
exposure to cold.

He was troubled with the Toothache
in a slight degree 15 years ago, which
soon went off, but returning about three
years ago, a carious tooth was extracted.

[Page 2]

Since the east & north winds sett in this
spring, has been attacked in a less (↑similar↑) mann¬
er tho' with less pain

Last year the pain was daily worse
about bed time, this year in the morning.
His pulse & appetite are good, belly open,
with occasional looseness, & at first ↑a slight↑ heat of
skin↑. The two upper grinders on the left
are somewhat loose, & have been so since
the attack last year, tho' in a greater deg¬
ree since the present attack; the basis
of the furthest, & of the corresponding one
below somewhat bluish, & on this side
the pain is extended along the ↑lower↑ gum, to
the middle of the mouth.

This spring leeches have been twice
applyed to the gums, first time with
relief, but last without success.

Soon after the last bleeding, I mean
the day following, a diffused swelling
appeared over the left cheek, without
relief of the pain: - it has never sub¬
sided, tho' not entirely removed. --

He is now taking the Elix. Guajac.
in small doses

[Page 3]

Last spring he was confined about a
month, & this year about 6 days.

I request at your convenience, your
opinion of this case.

Inclosed I have sent you a Guinea. --

Doctor
I am respectfully
your obedient humble servant

John Scruton

PS. By the patient I have had so much
{illeg}, and gnashing of teeth here ↑from this complaint↑, that I
hope to be excused hereafter.
