Keswick, Cumberland April 13th. 1779
Honoured Sir


The many Obligations and Favours you have been pleased
to confer upon me, with the kind offer you made me of your
Advice in any Cases of Difficulty I might meet with, are the
only Pretentions I have to request it in the following Case,
which would do me great Honour, and be great Satisfaction
to the Patient, who applied to me the Evening of my Arrival
in Keswick, of the Nature of whose Complaint I find myself
much in the Dark. --


A Gentleman aged 29, of a delicate weak Habit, who
has been a great Part of his Life pretty healthy, but being
obliged to lead a sedentary Life, and this last Winter much
exposed to Cold, to which he imputes the Cause of his Disorder,
was first affected about 7 Weeks since with an acute Pain
just under the false Ribs on the Right Side, which continued
without any other Symptoms for about a Fortnight, after which
it was succeeded with various shooting pains in the Breast
and Abdomen, resembling (if not) those of the Rheumatism,
which sometimes extended to the Top of the Shoulders, oc¬
casioning a soreness to the Touch ↑there↑; these also continued
about 2 weeks, when a Swelling of the Abdomen took Place;
then all the Pains gradually abated, except some slight ones
that yet remain at Times about the Kidnies. During all his
Disorder his Stomack has been as good as when in Health
and stools regular; but when he eats the Extension of the Belly
increases so preternaturally fast as to oblige him to desist.
Urine is high coloured and deposits a thick Brickish Sediment.
Pulse about 60 in a Minute, but weak. Sweats pretty much
in the Nights
, can only lay easy on the right side. The Scrotum
is also swelled and painful. He has taken an Emetic which also
proved a Cathartic; was ordered to take Ol. Terebinth & Sal. Nit.
but has found no Relief: He sleeps pretty well in the Nights
but the universal Swelling of the Belly increases. - -
