The case of Captain Corsan


Mr Corsan Febry. 1772 was affected with a Gonorrhea
attended as it would seem with considerable inflammation
of the Urethra as the ardor Urinæand Cordee were ↑was↑ violent
and sometimes blood was ↑very frequently↑ voided.


How this Disorder was managed we do not ↑know↑ exactly
but in general that a good deal of Mercury was given
that at the same time Mr Corsans own manner of living was very suitable to it.


Against the Month of August that year the Gonor¬
rhea
was gone andit appears to have been suddenly stopt and↑ but there remained some strangurious
affection, a frequent desire to make water with great diffi¬
culty in restraining it ↑some pain in passing it and↑ and much ardor passin it.
The Urine at same time ↑was↑ of a whey colour and on standing
depositinged a great quantiy of viscid whitish viscid fœtid
Mucus.


These symptoms better & worse at times have con¬
tinued ↑from August 1772↑ to the present day


From suspicion of venereal remains a good deal
of Mercury as an alternative was at first employed and
with it a Decoction of Sarsa.


At times a great variety of mucilaginous drinks
{illeg} have been used. For some time he drank the
Mineral water of Moffat and at the same time used very
often a warm bath.


At all times there has been a constant attention,
to keep his belly regular by the most wasy laxatives.

The measures mentioned were pursued for a twelve
month and frequently with seeming success as the symptoms
at times proved much easier but they were never entirely
gone and after a twelve month they were almost as bad as
ever.


He was then engaged in Cold Bathing which was con¬
tinued for a month with good effects on his general habit
but with none upon his particular ailment.


At the same time Bougies were employed and were
frequently introduced even into the bladder and were allowed
to remain introduced for a long time. When withdrawn they
were always covered over with a thick matter and what was
remarkable they appeared in several places as eroded, the plaister
being taken off so that the cloth was in these places bare.


These Bougies however were always introduced with
pain and by the repetition this increased so much that he
could not bear them. and At ↑the↑ same time the introduction became
more & more difficult and as they procured no relief of his
complaints the trial of them was entirely given ↑up↑. It is pro¬
bable that by making water without withdrawing the Bougie
while a Bougie remained in the Urethra he did a great deal
to hurt that passage and to prevent the easy introduction of
the Bougies afterwards.


X Bark & Cicuta


↑After all these has been employed without effect↑ He was now put upon a Course of Remedies with a
view to take of all swelling and inflammation of the urinary
passage
↑& to prepare for repeating the Bougies. Leeches were frequently applied to the Perinæum
He was engaged to lie very constantly in an horizontal posture
His belly was kept open. He abstained from meat and wine
↑His body was kept cool↑ He drunk plentifully of mild diluent & mucilaginous drinks

These measures for some time seemed to succeed very well
as all his symptoms became much easier but it was a
short truce only and they returned with as much or
more violence than ever so that at sometimes he was
threatned with a total suppression of Urine.


As the irritation to make water was especially
troublesome in the night and often so frequent as to
prevent sleep altogether, it became necessary to give
Opiates and the have been constantly continued since
so that his dose is now 130 drops of the Tinct. Thebaica every evening.


The constant use of these has rendered the use of
the laxatives also constantly necessary to be usefull for any
time. For a long time the Ol. Ricini with a little Spirits
answered the purpose extremely well but at length the
stomach came to refuse it. Salts & Manna have since
answered pretty well but these too have failed and he
is now using an Electuary of Crystals of Tartar with
a twelfth part of Jalop and one half pulp of prunes.


Thinking that somewhat might depend upon the state
of the Urine he for a long time took the Uva Ursi to
two or three drams a day but the effects have not been evident.


He has upon occasion tried a fomentation of the
Perinaeum and he has frequently had Oil injected into
the Urethra but neither with any benefit. ____

His present state is somewhat easier that it has
been at many other times but he has still much pain and
uneasiness. He has a frequent irritation to make water
has sometimes difficulty and pain in passing it and always [a?]
much heat & pain at the end ↑of it↑. His Urine has still little colour
and some mucus at the bottom tho not so much as formerly.


In this situation we think he has little chance for
relief but in the use of Bougies and think he should go
to London to have these applied as the surgeons there have
much more experience in these matters than any body here


In the meantime as his affairs require his remain¬
ing yet for a little time in this country we recommend
the following regimen to him.


He should keep to the same cool and light diet
as for sometime past. He should take very little animal
food and live for the most part upon milk & grain.


He must avoid all kind of strong drink but take
plentifully of mild diluent liquors


He will probably find it necessary to continue
his Opiate and therefore must also use a laxative every
day.


He should use very little bodily exercise when
he goes in a carriage it should be very gently and not
too fat at once.


He should upon all occasions avoid Cold and at
same time avoid keeping his body too warm either by warm
chambers or by many bedcloaths.
